Before you upload

Please name each file with your child’s full name, date of birth and document type. If the document shows a different name, such as “Baby of” or an earlier surname, please email our team with your child’s current details.

Referrals and supporting documents

Referral — for GP or specialist referrals.

Supporting document — for reports, results, discharge summaries or other documents requested by Neo2Paeds.
